APPLICATION FORMSAiken County HazMat Team Administrative Procedures require prospective members to submit a completed Membership Application. Additionally, per procedure and recent SC law a new member must have a criminal background investigation through local law enforcement and a SLED background investigation via a Firefighter Registration.Step 1 - Proceed to the Sheriff's Office or other SC law enforcement agency and request an "NCIC" Step 2 - Download and print each form below (requires free Adobe Acrobat Reader). Step 3 - Fill out these 2 forms; the Team application fully and also complete as much of the Firefighter Registration as possible. Step 4 - Submit the NCIC printout you obtained from the local law enforcement agency along with both the Team Application and Firefighter Registration to Lt.
